Vaccination Information - Fall 2017

****PARENTS OF CHILDREN GOING INTO 7th (need initial vaccine) and 12th (needs booster shot if 1st one was before 16th birthday) GRADES****
 
Your child/student must have their Meningococcal Vaccination or a booster to be able to attend school in the Fall 2017.
Proof must be provided (with Doctor Signature) BEFORE your child/student will be allowed to attend. 
 
*****Parents of children going into 6th grade must have up to date Tetanus (Tdap) and Varicella (chickenpox) immunization's********
