Community Health Assessment

The Cortland Community Assessment Team is in the process of completing the Cortland Counts Comprehensive Report to be published in 2019. Prior reports can be downloaded from the Seven Valleys Health Coalition website. The Community Assessment Team is comprised of the Cortland County Health Department, Cortland Regional Medical Center, United Way for Cortland County, SUNY Cortland’s Institute for Civic Engagement and Seven Valleys Health Coalition.  As part of the assessment process the public is asked to complete a Community Health Assessment survey: CortCHAsch2018
